New Jersey is a direct access state, which means licensed physical therapists can treat patients without seeing a physician. No referral necessary. Your digital intake form will be sent to you electronically after you call to make your first appointment.
Your driver's license, insurance card, and a prescription from your doctor if one was provided. Please fill out the new patient forms sent before your visit, or arrive 20-30 minutes early.
Approximately one hour, depending on your specific needs.
We are in-network with Horizon Blue Cross Blue Shield, Cigna, Aetna and Medicare, and we submit to all out-of-network insurance companies.
